Our MDC Community Health nursing service is provided on both an in-reach and outreach basis from each MDC and your clients will frequently be those who don't engage with mainstream community health services. In this senior clinical role you will focus on assessment and care coordination to ensure the delivery of best practice health care services to sexual assault survivors. As part of a small team and frequently working autonomously you will: 

  • Assist clients to transition into universal health care. 
  • Increase health literacy Provide health education. 
  • Develop and implement client-centred care plans. 
  • Identify client's needs through conducting holistic health assessments. 

About You

An experienced nurse with a passion for advancing patient care within a vulnerable client base, you have sound knowledge of the health care system and in particular the social model of health. You are an excellent communicator and work collaboratively with your team members and have a broad understanding of the needs of victims of sexual assault, family violence and their families. In addition you are committed to providing integration of care across the care spectrum and can demonstrate a high level of IT literacy. This role would suit an experienced Community Health or sexual and reproductive health nurse who has worked with a wide range of patients, who may be looking to increase their knowledge and expertise around trauma informed practice. Qualifications and/or experience in primary health, health education, case management, or women's health, mental health risk assessment, family violence and sexual and reproductive health will be advantageous.

What You Bring 

Working closely with SECASA counsellors, DFFH child protection, Victoria Police sexual offences and family violence investigation teams, VACCA you will bring the following demonstrated skills, experience and knowledge: 

  • Comprehensive assessment skills 
  • Understanding of the social model of health 
  • Ability to work with paediatric, adolescent and adult cohorts 
  • Sexual and Reproductive Health expertise (Desirable) 
  • ASSIST/Mental Health First Aid Training (Desirable) 
  • Understanding of Trauma Informed Care 
  • Family Violence Training (Desirable) 
  •

  • Self-awareness and emotional intelligence 
  • Skills in fostering relationships with external stakeholders 
  • Autonomy, workload management skills and initiative
