A new and exciting opportunity is now available for an experienced Occupational Therapist to join the team at Bolton Clarke, based out of our Ravenhall office in Melbourne's west. 

This is a part-time position with the ability to work from home. This home and community care role will provide clinical support services to our valued clients in their own homes, enabling them to stay at home and remain independent for as long as possible. All the tools of the trade will be provided, including a company smart phone and laptop.

The ideal candidate will possess exceptional analytical, problem solving and negotiation skills with the ability to seamlessly manage varying and/or conflicting stakeholder demands. 
  
Duties:

  • Conduct home based assessments including all areas of activities (daily living, mobility and home safety).
  • A focus on falls prevention and contribute to the continuous improvement to the overall program.
  • Develop care plans in consultation with the resident and their family to ensure we are providing the highest quality of care for each and every client.
  • Work to foster productive working relationships with relevant government departments, hospitals, key providers and other specialist providers including network with occupational therapy services and other practitioners within the region.
  • Previous relevant experience within an Aged or Community care environment.
  • Strong understanding of all relevant legislative, regulatory requirements, professional standards and guidelines relevant to the Aged Care sector (desirable). 

Qualifications: 

 

  • Bachelor Degree in Occupational Therapy or equivalent.
  • Current Occupational Therapist Registration with AHPRA.
