Service Facilitator

Position Overview:
As a Service Facilitator, you will be at the forefront of empowering our Participants and promoting their well-being. Working closely with Support Workers, individual Participants, and their families, you will play a pivotal role in implementing personalised care plans and ensuring their effective delivery.
Additionally, you will provide guidance and support to our dedicated Support Staff, fostering a culture of excellence and ensuring the highest quality of care. Your contributions will extend beyond daily operations as you actively engage in the continuous improvement of our services.
Key Responsibilities:
Client Support:

Work closely with Participants to develop individualised support plans, tailored to their specific needs and preferences.

Conduct comprehensive assessments of participant’s needs related to their medical conditions

Provide therapeutic interventions aimed at adaptation throughout the continuum of care.

Identify and evaluate factors affecting participant’s health, including family dynamics, mental heal, and socio-economic challenges
Documentation:
-
Maintain accurate and up-to-date records of resident information, service plans, and incident reports.

Maintain accurate and timely documentation of participant’s assessments, interventions, and outcomes.

Prepare reports and summaries for case reviews and multidisciplinary team meetings.
Stakeholder Collaboration:

Collaborate with NDIS agencies, healthcare professionals, and external partners to ensure effective and coordinated support services.

Maintains liaison with other health care facilities, community agencies, and other appropriate organizations as required in order to build relationships and assist participants with access to community programs and services
Service Delivery:

Ensure that support services are provided in a person-centered and respectful manner, respecting the individual's rights and choices.

Provides education to patients and families around issues related to adaptation to the participant’s diagnosis, illness, treatment, and/or life situation.
Other General Responsibilities:
Site Management: Oversee the day-to-day operations and management of multiple Supported Independent Living sites.
Team Leadership: Supervise, mentor, and provide guidance to a team of support staff to ensure the highest level of care and support to residents.
Compliance: Ensure that all services provided comply with NDIS standards, regulations, and quality assurance requirements.
Budget Management: Assist in budget planning and management for each SIL site.
Quality Assurance: Implement quality assurance measures to assess and improve the quality of services offered at each site.
Emergency Response: Develop and implement emergency response plans and protocols to ensure residents' safety.
Qualifications:

Diploma in social work, human or community services, or a related qualifications will be highly regarded.

Proven experience in a similar role, with a minimum of 2 years in NDIS SIL management.

Strong leadership and team management skills.

Excellent communication, negotiation, and problem-solving abilities.

In-depth knowledge of NDIS regulations, standards, and best practices.

Strong computer literacy, including proficiency in Microsoft Office.
