As a Refugee Health Nurse, you’ll provide direct patient care, advocate for access to services, and collaborate cross-culturally to address health disparities and promote wellness. Reporting to the Clinical Nurse Coordinator, this is a full-time, ongoing role (1.0 FTE).
Your Passion and Commitment will see you:
- Conduct initial health and social needs assessments, care planning, and case conferencing for new arrivals, following the YourCH practice model and guidelines
- Coordinate client healthcare by liaising with services, schools, and a range of providers (GPs, dental, nutritionists, school nurses, trauma counselling, hospitals, etc.), including facilitating access to interpreters and transportation support
- Involve clients and carers in decision-making and collaborate to negotiate agreed care plans
- Work closely with local settlement services to quickly address the needs of newly arrived individuals
- Promote social connection and integration by referring clients to support and orientation programs
To succeed in this role you will have:
- AHPRA Division 1 Registered Nurse, with full and unrestricted registration
- Minimum of 2-3 years' experience in primary health care settings or as a Refugee Health Nurse
- A strong understanding of trauma-informed care and cultural safety
- Demonstrated understanding and respect for diverse cultural backgrounds, traumatic experiences, and unique health challenges
- Lived experience and ability to speak a relevant community language (desirable)
