This is a volunteer role on the Pathways Specialist Nurse (Telehealth) service. Training and ongoing support is provided and hours are flexible. Working from home, you will need to be available for a minimum of three patients per week over a 12 month period. 

Using an access model of care, the Pathways Specialist Nurse plays a crucial role in delivering high-quality, patient-centered care through virtual platforms. This role involves using clinical expertise to assess, plan, and coordinate care for patients, ensuring they are not merely guided through appropriate healthcare pathways, but able to access the care they need. The nurse acts as a liaison between patients, families, and multidisciplinary care teams, ensuring continuity of care and adherence to evidence-based guidelines.

Key Responsibilities:

  • Patient Assessment & Care Coordination

    • Conduct virtual consultations with patients to assess symptoms, medical history, and care needs.
    • Utilize clinical decision support tools to determine appropriate care pathways.
    • Triage patients based on severity of symptoms, directing them to the appropriate level of care (e.g., primary care, urgent care, or emergency services).
  • Care Access

    • Assist patients in understanding and accessing available healthcare services.
    • Provide guidance on follow-up care, diagnostic testing, or referrals to specialists.
    • Ensure smooth transitions between telehealth and in-person care when necessary.
  • Clinical Documentation

    • Accurately document patient interactions and care plans in electronic health records (EHR) systems.
    • Maintain compliance with telehealth regulations and organizational policies.
  • Patient Education & Advocacy

    • Provide education on disease management, medication adherence, and preventive care strategies.
    • Advocate for patient needs and preferences in care planning.
  • Quality Assurance & Improvement

    • Participate in the evaluation of telehealth services, identifying areas for improvement.
    • Stay updated on telehealth best practices and integrate evidence-based approaches into patient care.
  • Collaboration & Communication

    • Work closely with physicians, advanced practice providers, and other healthcare professionals to develop care plans.
    • Act as a point of contact for patients and families, ensuring timely communication about care plans and next steps.
