The Registered Nurse is responsible for the delivery of high quality, safe care to the practice population that is consistent with current best practice standards. The role involves working closely with all members of the clinical team to ensure optimum patient care/outcomes.

 

Essential Criteria

  • Current registration with Australian Health Practitioner Regulation Agency as Registered Nurse Division 1
  • Appropriate credentialing and competence suitable for the delivery of care in general practice
  • Experience in caring for patients with complex needs
  • Demonstrated excellent and effective interpersonal and organisation skills
  • Demonstrated commitment to ongoing professional development
  • Demonstrated involvement in quality improvement initiatives
  • Ability to work collaboratively and independently
  • Computer Literacy

 

Desired Criteria

  • Previous experience in General Practice or Primary Care
  • Experience in chronic disease management
  • Nurse Immunisation Accreditation
  • An understanding of the Medicare Benefits Schedule
  • An understanding or experience of the Royal Australian College of General Practitioners Standards for General Practice

 

KEY PERFORMANCE AREAS

Clinical Care

  • Works within own scope of practice and competence to provide comprehensive clinical care to all patients both within the practice and in outreach settings (for example, nursing home, retirement village)
  • Provide episodic and ongoing care that is responsive to patient circumstances
  • Collaborates with General Practitioners to deliver appropriate patient care and evaluate the outcome of the same
  • Provides ongoing monitoring and support of patients with chronic and complex health issues
  • Coordinates and runs nurse led clinics for chronic diseases and health prevention and maintains an accurate recall and patient register
  • Ensures regular follow up of patients enrolled in Coordinated Veterans Care (CVC) program
  • Provides patient (and family) education on preventative health strategies and treatment/care plans
  • Uses best available evidence to inform clinical care
  • Actively participates in case conferences
  • Plans and prioritises workloads to meet patient needs

 

Professional Development/ Education

  • Operates within all professional standards applicable to the Nursing and Midwifery Board of Australia, including its registration standards, professional code of conduct, guidelines and scope of practice
  • Works with the Director and Management to develop and maintain practice policies and procedures in line with accreditation and standards for general practice
  • Participates in weekly multidisciplinary clinical meeting
  • Takes responsibility for own professional development and learning
  • Identifies need to update knowledge base for practice
  • Attends/participates in both formal and informal educational programs
  • Provides supervision and/or support of more junior clinical staff (for example, nursing and medical students) as required
  • Contributes to the ongoing education of staff
  • Able to report accurately and in a timely manner to management and the Director

 

Safety and Quality Management

  • Practices duty of care and maintains patient and practice confidentiality
  • Ensures consumables, equipment and medications are kept stocked and maintained to the manufacturers standards and conditions, including the safe storage, rotation and disposal of drugs and vaccines as well as a register of stock and asset register

  • Ensures that all practice equipment is operational via the completion of practice policy checks
  • Maintains infection control standards within the practice
  • Participates in risk management and quality improvement processes and ensures clinical governance processes are in place
  • Participates in general practice accreditation processes
  • Recognises and works within own level of competence
  • Maintains accurate, contemporaneous and complete patient records consistent with legal requirements and organisation policy
  • Maintains and monitors clinical rooms and work areas to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies and guidelines
