Osana is a new type of health care in Australia. Our mission is to keep patients well, out of hospital where possible, and help them to live healthier. We will do what other parts of the health system may not do – take responsibility for your health outcome.

We are setting up clinics in Sydney, opening in 2018. These clinics will provide usual GP medical services, but specialise in helping Australians with chronic health conditions. These clinics will provide:

• A bus to transport patients to appointments
• Frequent home visits by staff from the same clinic
• Co-located allied health clinicians who coordinate your care
• In house exercise, cooking and education classes
• Health coaches who attend appointments with you
• Telehealth support (ask your GP questions via email, phone or video)
• Dedicated health plans with specialist input to ensure the best healthcare possible.

Doctors, nurses, physios, dietitians, psychologists and health coaches will spend more time with you to support you towards better health. The experience aims to be completely new, and hopefully, life changing for patients.
