Floreat Surgery
Employer Business Type
Profile Summary
Floreat Surgery is now a mixed billing practice and our Independent GP's may no longer direct bill patients to Medicare routinely.
Consultations are made by appointment and are 15 minutes per patient to assist with 1-2 concerns.
Longer consultations for multiple problems, insurance and employment medicals, removal of lesions, first antenatal visits, and full “medical†checks can be booked for 30 minutes.
Please report to the reception desk upon arrival and our staff will check your identifiers by asking for you full name, date of birth and address. Please advise changes required to your change of address, phone contact including mobile, Medicare or Centrelink entitlements and next of kin.
Urgent matters e.g. chest pain, trouble breathing, feverish child, and allergic reactions need to be dealt with as a priority and may effect your waiting time. We apologise for any inconvenience.
Test results, recalls, repeat prescriptions and referrals require a booked appointment.
Telephone consultations are available, if eligible, by booking only, fees apply.
New patients require a 30 minute appointment with the nurse followed by 30 minutes with the GP.
If you are unable to keep your appointment, please notify us as early as possible so that the appointment can be re allocated. A sms will be sent on failure to notify us of a cancelled appointment and may incur a fee.
