Perth Diabetes Care is a patient centred multidisciplinary clinic providing services for people living with all types of diabetes and chronic conditions.

Our health professionals include Diabetes Educators, Dietitians, Exercise Physiologists, Counsellors and Podiatrists. They use current best practice guidelines and state of the art technology to provide individualised health care and education to support the person living with a chronic condition achieve their health goals.

We have a range of different services available including individual appointments and group education.

We are a private billing clinic but you may be able to access rebates through Medicare, Private health insurance and insulin support programs if you are eligible.
