Support and care planning at the end of life
Session overview
Description
This session aims to outline the role and importance of good support and care planning for a person and their carer.
This session was reviewed by Tes Smith and last updated in September 2024.
Learning objectivesBy the end of this session you will be able to:
- describe your awareness of end-of-life issues
- understand the role of support and care planning at the end of life
- understand the role of advance care planning
- be aware of ways to approach talking to people and their carers about death and dying
- be aware of your impact as a worker on the support you offer individuals and their carers
Before commencing this session you should complete:
- The relationship between palliative care and end-of-life care (215-0002)
- Supporting people to live and die well (215-0601)
In the UK, we are seeing a number of changing trends which are having an impact on end of life care and dying:
- life expectancy is increasing
- more older people are living alone and families are more dispersed
- there are many more people living with multiple chronic conditions
- more people are hospitalised at the end of life. Therefore, less end of life care is provided in the community and there is less experience of death and dying in the home
This session aims to outline the role and importance of good support and care planning for a person and their carer.
As this module requires reflection on end-of-life issues and our own responses to difficult situations, please be aware of the impact this may have on you, and seek help to talk about your feelings with a trusted friend or colleague during your studies.
