Transition from Child-centred to Adult-centred Healthcare for Paediatrics professionals
This session explores the importance of transitional care, as young people with long-term conditions and/or disability negotiate the move from child-centred to adult-orientated care.
Learning Objectives
By the end of this session you will be able to:
- Describe how transitional care is a part of adolescent healthcare
- Describe the philosophy of transition and distinguish it from transfer
- List examples from the evidence base to support transition
- Describe the differences between paediatric and adult healthcare cultures and the different perspectives of the key people involved
All young people with a long-term condition and/or disability require developmentally appropriate health care, transitional care is a key element of this. It is important to recognise that this session is not only about young people attending paediatric services but also those young people who present for the first time to adult services who also need both developmentally appropriate health care as they acquire the skills to navigate adult healthcare.

Dr Janet E McDonagh is a Clinical Senior Lecturer in Paediatric and Adolescent Rheumatology at the Centre for Musculoskeletal Research at University of Manchester and Honorary Consultant paediatric and adolescent Rheumatologist at Royal Manchester Children’s Hospital.
Her main clinical and research interests are in adolescent health and transitional care. She has a particular interest in models of care for adolescents with chronic conditions that support young people’s emerging capacity for self-management, that ensure appropriate health risk assessment, support transition to adult health care, adherence to treatment guidelines and engagement with other health care services.
She is a member of the Barbara Ansell National Network for Adolescent Rheumatology, convenor of the Royal College of Paediatrics and Child Health Young Person’s Special Interest Group, co-chair of the Royal College of Physicians Young Adult and Adolescent strategy group and is on the advisory council of the Association for Young People’s Health.
Dr McDonagh is also involved in the development of training for health professionals in adolescent medicine including her role as a trainer in the European Teaching Effective Adolescent Care and Health initiative
Clinically active with interests in adolescent health as Adolescent lead for development of adolescent services in UHCW and as a Community Paediatrician caring for teenagers with severe learning disability in the community setting and working with colleagues in the Learning Disability setting to facilitate transition. Member of the RCPCH Young people’s Health Special Interest Group.
Prior to her recent retirement Marcelle de Sousa was an Adolescent Nurse Specialist in the Adolescent Unit at University College Hospitals in London. She has been involved in transitioning young people to adult services since 1986 when she was a paediatric renal nurse. She was the chair of the Adolescent Health Forum of the RCN and the regional member of the London Board of the RCN. Marcelle has been a member of the Children’s Health Alliance Adolescent subgroup and was one of the Department of Health Transition Champions.
