This session considers the practical aspects of timing and planning of transition and transfer for young people with long term conditions and/or disabilities.

 

Learning Objectives

By the end of this session you will be able to:

  • Describe the principles of transition planning and timing of transfer
  • Describe core knowledge and skills integral to transition and self-management skill development for adolescents with long-term conditions and/or disabilities
  • Demonstrate methods in person-centred transition planning with young person, parent and other professionals and agencies

 

Transitional care is now advocated by national professional organisations in the UK, USA and Canada but challenges remain as to how such policies are translated into practice.

Integral to transition is young person-centred planning which addresses the medical, psychosocial and educational/vocational issues facing the individual young person as they grow up and eventually move out of child-centred services. This session will explore the key components of this planning process.

Janet McDonagh,
Clinical Senior Lecturer in Paediatric and Adolescent Rheumatology at the Centre for Musculoskeletal Research at University of Manchester

Dr Janet E McDonagh is a Clinical Senior Lecturer in Paediatric and Adolescent Rheumatology at the Centre for Musculoskeletal Research at University of Manchester and Honorary Consultant paediatric and adolescent Rheumatologist at Royal Manchester Children’s Hospital.

Her main clinical and research interests are in adolescent health and transitional care. She has a particular interest in models of care for adolescents with chronic conditions that support young people’s emerging capacity for self-management, that ensure appropriate health risk assessment, support transition to adult health care, adherence to treatment guidelines and engagement with other health care services.

She is a member of the Barbara Ansell National Network for Adolescent Rheumatology, convenor of the Royal College of Paediatrics and Child Health Young Person’s Special Interest Group, co-chair of the Royal College of Physicians Young Adult and Adolescent strategy group and is on the advisory council of the Association for Young People’s Health.

Dr McDonagh is also involved in the development of training for health professionals in adolescent medicine including her role as a trainer in the European Teaching Effective Adolescent Care and Health initiative

Anita Morgan,
Consultant in Acute and Community Paediatrics at UHCW and Gulson Hospital Coventry

Clinically active with interests in adolescent health as Adolescent lead for development of adolescent services in UHCW and as a Community Paediatrician caring for teenagers with severe learning disability in the community setting and working with colleagues in the Learning Disability setting to facilitate transition. Member of the RCPCH Young people’s Health Special Interest Group.

Marcelle Sousa,
Adolescent Nurse Specialist (now retired)

Prior to her recent retirement Marcelle de Sousa was an Adolescent Nurse Specialist in the Adolescent Unit at University College Hospitals in London. She has been involved in transitioning young people to adult services since 1986 when she was a paediatric renal nurse. She was the chair of the Adolescent Health Forum of the RCN and the regional member of the London Board of the RCN. Marcelle has been a member of the Children’s Health Alliance Adolescent subgroup and was one of the Department of Health Transition Champions.
