Learning objectivesBy the end of this session you will be able to:
- demonstrate an understanding of the Framework for Social Care
- recognise the importance of social care in end of life care
- demonstrate an understanding of how social care can contribute to end of life care
- use the framework to identify some personal development priorities
The NHS National End of Life Care Programme 'Supporting People to Live and Die Well: A Framework for Social Care at the End of Life' (2010) document is intended to signal a change in how social care engages with, and delivers, end of life care [1](read details regarding the reference).
It represents an invitation to the social care and healthcare communities (and other stakeholders) to work to improve understanding of what works in practice and what resources and tools may be available, and to help build a network of champions who can drive forward the necessary change.
Of note, the document does have a section on end of life care pathways. This is now obsolete, and care in the last days of life should focus on the '5 priorities of care' developed by the Leadership Alliance for Care of Dying People [2](read details regarding the reference).
