SESSION ONE:
Demystifying Dementia: Diagnosing Mild Cognitive Impairment and Dementia
Learning outcomes:
- Be able to provide timely identification and acknowledgment of cognitive impairment or dementia.
- To efficiently collect patient and informant history, physical examination, cognitive assessment and functional assessment to identify cognitive impairment or dementia.
- To understand how information to identify cognitive impairment or dementia can be collected across multiple visits, by the primary healthcare team.
SESSION TWO:
Demystifying dementia: supporting people with MCI and dementia to live well
Learning outcomes:
- To be able to share a diagnosis of mild cognitive impairment and dementia with the patient and family.
- To be able to plan a post –diagnostic framework for a series of patient reviews over 6-8 weeks that addresses:
- To be able to plan follow-up that includes assessment of cognitive, functional performance and review of planned support strategies.
- To be able to present an economically sustainable model within their practice that supports timely diagnosis and good care for MCI and dementia.
SESSION THREE:
Facing Dementia in practice – practical tips for tricky issues
Learning outcomes:
- Have difficult conversations around dementia such as suggesting cognitive assessment, managing driving, capacity and EPOA planning and end-of-life.
- Understand how to work with other general practice team members and specialists in diagnosing and managing dementia.
- Understand some aspects of the diagnosis and management of later more complex complications of dementia such as BPSD, motor dyspraxia and sarcopenia.
