People with long-term conditions should have a plan of care to help them self-manage their condition/s. This session looks at the different approaches and designs of care plans, so that you can assist patients with enduring mental and/or physical health problems.

Learning Objectives

By the end of this session you will be able to:

  • Describe how helping a patient plan their care can promote self-management
  • Expalin the best way to organise the consultation to enhance the care planning process
  • Describe the consultation approaches which will enhance the care planning process
  • List the different types of care plans available
  • Help a patient with a long-term condition(s) plan their care

Using a care planning approach in which the patient and the health professional (and carer if appropriate) plan and review care regularly can be effective in improving patient outcomes and quality of life.

Sheila has 20 years of experience in primary care where her clinical work included caring for patients with physical and mental health problems.

Sheila is keen to improve the care of people with mental health problems in primary care. She has carried out research to show the benefits of practice nurses monitoring the physical health in people with serious mental illness (SMI), and how using a psychological intervention in patients with chronic obstructive pulmonary disease can promote self-management.

Sheila writes educational articles for practice nursing journals and has co-authored a book about mental illness in primary care. She has created training for practice nurses in various areas of mental health.
