About the session
This is a training session to learn about the importance of person-centred care and how shared decision-making and personalised care planning can improve compliance and improve health outcomes.
At the end of this session you will be able to:
- recognise the importance of person-centred care and approaches to utilise in practice
- differentiate between a people’s definition of frailty as opposed to an academic version
- recognise things to consider about living well with frailty
- analyse the importance of how we share information and what we need to consider
- interpret shared decision-making and personalisation of care plans to ensure greater compliance with recommendations that are jointly agreed with the person
- recognise the importance of time within frailty
- recognise the importance of safeguarding
